What if something abnormal turns up during the bimanual exam? The bimanual exam gives a relatively accurate indication of the size and position of your organs and what is happening in the larger physical sense, but it does not reveal the inner composition of anything unusual. I may feel an enlarged ovary, but I am unable to tell from the physical exam whether it is a cyst filled with fluid, perhaps from endometriosis, or a solid mass that might be a cancerous tumor. To get this kind of information, I might recommend an ultrasound study, because this kind of imaging can distinguish between solid and fluid-filled growths.

Changes in diet that bring about significant weight gain can also cause your periods to cease temporarily, as can drastic changes in exercise pattern. If you have been a couch potato and suddenly start running ten miles a day, you may very well stop menstruating for a time. Tension can interfere with your menstrual cycle in a major way. If you are under heavy stress at school or have recently gone away to college, you may have irregular periods—or no periods for a while. An illness in the family or a death can have the same effect.

If this is your first exam, the doctor will use a small one. If you have had several children, a wider one is appropriate. Your physician will insert the speculum gently into your vagina and encourage you to relax. Try to stay as loose as you can. If you are tense, your doctor will probably give you a little extra time. The cervix is in different locations in different women. Sometimes it just pops into view once the speculum is opened, but sometimes it is wedged high up in the vagina and is difficult to find.
