In addition, obstetricians who provide care for high-risk patients, maternal–fetal medicine specialists, and neonatologists are unevenly distributed among geographic areas and types of facilities. Good data are lacking on the number of obstetricians who provide care for high-risk patients and the number of neonatologists needed to serve a given population. A team approach to perinatal health care delivery is essential to improve the outcome of pregnancy. Certified nurse–midwives, certified midwives, laborists, hospitalists, family practitioners, physician assistants, advanced practice registered nurses, respiratory therapists, perinatal social workers, lactation consultants, and other professionals also are important health care providers of perinatal services.
In these units, the nurse also should be able to provide care for infants requiring inhaled nitric oxide therapy and high-frequency ventilation as well as care for the chronically technology-dependent infant. An advanced practice registered nurse should be available to the staff for consultation and support on nursing care issues. Additional nurses with special training are required to fulfill regional center responsibilities, such as outreach and transport (see also “Transport Procedure” and “Outreach Education” in Chapter 4).
These team members work together, utilizing mutually agreed upon guidelines and policies that define the individual and shared responsibilities of each member. Although the responsibilities of obstetrician–gynecologists place them in the role of ultimate authority because of their education and training, the contributions of each team member are valued and important to the quality of patient outcomes. The concept of a team guided by one of its own members and the acceptance of shared responsibility for outcomes promote shared accountability.
